Quick Links: One Year Executive Summary
This Abbott funded collaboration between Kids Hope United and the Lake County Health Department provides comprehensive services during pregnancy and the first three years of a child's life.
The lowest income/highest risk new mothers and their children receive support and intervention through home visits from highly trained professionals. Referrals are made through the Health Department who provide the services for parents/caretakers of three to five year olds, and an infant or child on the way, with identified risk factors.
This expanded Home Visiting program is only active in the Waukegan community. Through this partnership, the neediest families are served so that other important programs
can provide important services to families with lesser/varying needs.
